In the Defense Forces of Ukraine, the electronic primary medical record (ePMR) is being scaled up, enabling medics to work with data on wounded soldiers within a unified digital system during evacuation.
According to Oboronka, this was reported by the Ministry of Defense.
“ePMR is an electronic document that accompanies the wounded from the point of first medical aid to the hospital, transferring medical data into a unified digital system. It is already used in more than 250 units and continues to scale up,” the statement said.
It is noted that now any medic at a stabilization point, surgical department, or mobile team can enter injury data via the MedEvac application.
The charitable foundation “Come Back Alive” previously provided Defense Forces medics with 4,060 tablets, enabling the use of the application in which the ePMR system is implemented.
The statement emphasizes that information about injuries is recorded electronically, stored in a unified system, and is accessible to medics at every stage of care.

In particular, data is not lost during evacuation, and doctors at hospitals know the servicemember’s condition before their arrival. This reduces the risk of errors and repeated procedures, while records are standardized and easy to read.
It is also noted that the system allows medics to spend less time on paperwork and more time on providing care.
Through the MedEvac application, a medic enters data on the servicemember, their clinical condition, injuries or illnesses, the care provided, administered medications, and further evacuation details.
This information is transmitted to the Armed Forces of Ukraine Medical Information System, enabling medics at the next stage of evacuation to assess the patient’s condition in advance and prepare for their admission.
The statement also claims that the electronic primary medical record serves as official confirmation of injury for passing the military medical commission and receiving payments.
The electronic primary medical record (ePMR) in the Defense Forces was introduced in February 2025.
Earlier, it was reported that the First Separate Medical Battalion is implementing its own digital system, “Sisyphus,” for collecting, processing, and analyzing data on the wounded, evacuation, and resource use.
